newpatients
Our clinic is accepting new patients. To schedule an appointment, call 936-788-6060. As a new patient, you will be required to fill out a patient information form (these forms can be found under the Patient Forms tab and should be downloaded and filled out before your visit and brought with you at the time of the visit). The day of the first visit, please arrive 15 minutes before your scheduled time and bring with you the following forms:
Complete Registration Packet
Completed Health History
Insurance Card [primary and secondary, if applicable]
Drivers License / Picture ID for parents / guardians
Complete Immunization Records (first visit and each Well Child visit).
Initial Newborn Visit – Newborn record from hospital
If you do not complete the required paperwork before your office visit, please arrive 30 minutes before your scheduled appointment.

Please arrive 10 minutes before your scheduled appointment. Be sure to bring your current insurance card and notify the receptionist of any changes to insurance, address, or phone numbers.
Well Child and Physical exams require:
~ Immunization records.
~ School / Sports / Camp / Scout Physical forms with parent information completed upon arrival for appointment.

copayments
Parents are expected to present the current insurance card at each visit. All co-payments and any balances on accounts are due and payable at the time of service.
Child Custody / Divorce Cases – It will be the responsibility of the parent or guardian that brings the child into the Clinic for treatment –
To bring a current insurance card to each appointment
To pay all co-pays, deductibles and balances on account. It is the parents’ obligation to work out an agreement between themselves or through the court system to share the cost of medical services for their children.
cancellationpolicy
In order to minimize the waiting time for our patients, we reserve the right to reschedule your appointment if you are more than 20 minutes late.
If you are unable to keep your child’s appointment, please call our office as soon as possible, preferably 24 hours in advance, to cancel and / or to reschedule. This policy helps us to offer appointment times as they are requested by all patients.
If you miss two appointments in a row without any prior notification, you will be charged $30. If you miss a third appointment, you may be subject to discharge from our practice.
prescriptions
Please bring a complete list of your child’s medications on his / her first visit so that we can create a medication profile. Our office will then have access to your child’s Medication Profile for continued care.
Please let your physician / nurse know of any medication refills that you may need during the time of your clinic visit. If you are requesting a refill at other times, you may call your pharmacy first and they will fax us a request for approval.

If you are calling after clinic hours because you feel that your child needs immediate medical attention – please have ready:
Child’s name, date of birth, current weight
Name of current prescription medications – dosage, how often your child is taking it
Name, location and phone number of a 24-hour Pharmacy near you

St. Luke’s Episcopal Community Medical Center – the Woodlands
Memorial Hermann Hospital – the Woodlands
diagnostictesting
We utilize the services of Quest and Labcorp Laboratories in our area. We also use Aurum Diagnostic Imaging Center for X-rays and Ultrasound testing. We can also use any of the above hospital outpatient services for these tests, as well.
walkinpolicy
Our policy is that we see patients by scheduled appointment times. If you do walk in without an appointment, a triage nurse will assess your child.
If your child is assessed to be in an unstable condition, you will be directed to the nearest emergency facility for further evaluation.
If your child is in stable condition, you will be given an appointment at the first available time that day. If you cannot wait, you may make an appointment for a later time that day or for a different day.

A licensed Triage Nurse is available during usual office hours for routine questions, prescription refills, and for deciding if your child needs an appointment to be seen. All efforts will be made to schedule your child at the earliest available time slot to meet the needs of your child. We always try to return your call as soon as realistically possible. Please be available for her call-back with the child’s temperature [ if she/he has a fever ], latest weight, and the phone number for your preferred pharmacy.
